Pioneer 3 launched 1958


In a string of failed missions as part of the inauspicious beginning of NASA, the US space program, Pioneer 3, launched December 6, 1958, did not reach the moon as planned and fell back to Earth the following day.  It did manage to attain an altitude of about 63,600 miles and was able to measure radiation in the outer Van Allen Belt, so although its original mission failed, the flight did send back meaningful data.
